What changes is this bill making?
  1. 1Authorizes Medicare to add 2,000 residency slots each fiscal year from 2026 through 2032, with any unused slots carried forward to the next year.
  2. 2Reserves one third of the slots annually for hospitals already training above their current cap that train at least 25 percent of residents in primary care or general surgery.
  3. 3Establishes annual application rounds; hospitals apply and are notified by January 1 of each year, with new slots effective July 1.
  4. 4Requires the Centers for Medicare & Medicaid Services to consider a hospital’s likelihood of filling slots and to reserve at least 10 percent of positions for rural hospitals, hospitals over cap, states with new medical schools, and hospitals in health professional shortage areas, giving extra priority to those affiliated with historically Black medical schools.
  5. 5Limits each hospital to a maximum of 75 additional positions from 2026 through 2032, unless unclaimed slots justify raising that cap.

Resident Physician Shortage Reduction Act of 2025This bill increases the number of residency positions eligible for graduate medical education payments under Medicare for qualifying hospitals, including hospitals in rural areas and health professional shortage areas. The bill provides for an additional increase of 2,000 positions per fiscal year from FY2026-FY2032; during this period, each hospital may receive up to 75 additional positions in total under the bill and current law. Additionally, one-third of the positions that are made available under the bill must be allocated to hospitals that are already operating above applicable resident limits. The bill also requires the Government Accountability Office to report on strategies to increase the diversity of the health professional workforce, including with respect to representation from rural, low-income, and minority communities.
